I would interpret the two sentences as follows:
- He told her that he loved only her.
He told her that there is nobody else whom he loves.
- He told her that he loved her only.
He told her that the only thing he feels for her is love.
I would actually say that 5. is ambiguous:
- He told her that he only loved her.
I could interpret this as having the same meaning as either 6. or 7.
I disagree with Joanna Weselby, as I see a distinct difference between 6. and 7.

Both 5 and 7 are ambiguous, depending on where the stress is when said. «He told her that he only loved her» means the same as «He told her that he loved only her» = same as 6. «He told her that he only loved her» means the same as «He told her that he loved her only» means that the only thing he feels for her is love.

Actually, adding to my previous comment, you can change the meaning of 7 again with yet another stress: «He told her that he loved her only» which has the same meaning as 4. Stress on he doesn’t change the meaning for 5 though.

Это два отличных примера использования косвенной речи или reported speech. Без нее язык был бы явно неинтересным и слишком топорным. Так что давайте разберемся, что же reported speech такое и как ее использовать .
Итак, согласно толковому словарю слово «косвенный» означает «непрямой, побочный». Соответственно, косвенная речь – это непрямая речь, передача чьих-то слов. Зачастую её именно так и называют – непрямая речь (indirect speech). В отличии от русского, при переводе прямой речи в косвенную, в английском языке действует правило согласования времен, которое заключается в следующем:
Если глагол (действие) в главном предложении стоит в прошедшем времени, то глагол (действие) в придаточном предложении не может употребляться в форме настоящего или будущего времени, он должен быть выражен одним из прошедших времен.
Прямая речь: I love English. — Я люблю английский.
Косвенная речь: She said that she loved English. — Она сказала, что любит английский.
She said – главное предложение.
She loved English – придаточное предложение.
Обратите внимание: при переводе косвенной речи на русский язык, глагол остается в том же времени, что и в прямой речи. То есть, в приведённом выше примере она сказала, что именно «любит», а не «любила», как вам, может быть, захотелось перевести.
Согласно простому правилу, все времена в придаточном предложении сдвигаются на одну ступеньку вниз:

Present Perfect (have/has done) — Past Perfect (had done)

He said he had lost his keys. — Он сказал, что потерял ключи. -
Will — Would
I will go to this party. — Я пойду на вечеринку.
He said he would go to the party. — Он сказал, что пойдёт на вечеринку. -
Can/May/Must/Should — Could/Might/Had to/Should
I can do it. — Я могу сделать это.
He said he could do it. — Он сказал, что может сделать это.
Но меняются в предложении не только глаголы, а еще определения и обстоятельства:

I need this job. — Мне нужна эта работа.
He said he needed that job. — Он сказал, что ему нужна та работа. -

She said that he was dancing then. — Она сказала, что он тогда танцевал. -
Today — That day
I am going to do it today. — Я собираюсь сделать это сегодня.
He said that he was going to do it that day. — Он сказал, что собирался сделать это в тот день. -
Yesterday — The day before
I did it yesterday. — Я сделал это вчера.
He said he had done it the day before. — Он сказал, что сделал это за день до этого. -
A year ago — A year before
I saw him a year ago. — Я видела его год назад.
She said she had seen him a year before. — Она сказала, что видела его за год до этого. -
Last night — The previous night
I was there last night. — Я была там прошлой ночью.
She said that she had been there the previous night. — Она сказала, что была там предыдущей ночью. -
Tonight — That night
I will do it tonight. — Я сделаю это сегодня ночью.
She said that she would do it that night. — Она сказала, что сделает это той ночью.
Если же глагол в главном предложении стоит в настоящем времени, то правило согласования времен не соблюдается:
- He says that he wants to eat that sandwich. — Он говорит, что хочет съесть тот сэндвич.
- He says that he ate that sandwich yesterday. — Он говорит, что съел тот сэндвич вчера.
- He says that he will eat one more sandwich later. — Он говорит, что съест ещё один сэндвич позже.
Подводя итог: главное, что нужно запомнить при переводе косвенной речи в прямую – сдвиг на ступеньку вниз, если глагол в главном предложении стоит в прошедшем времени. Не так уж всё и сложно, правда? 😉

В косвенной речи основной глагол (Paul said that … / I told her that … и др.) и остальная часть предложения обычно находятся в прошедшем времени:

That можно и не использовать, т.е. вы можете сказать:

Как правило, форма настоящего времени в прямой речи изменяется на форму прошедшего времени в косвенной речи:

have/has → had
will → would
can → could
want/like/know/go etc. → wanted/liked/knew/went etc.

Хорошо, что в английском языке много глаголов, которые помогают передавать чьи-то слова, иначе речь звучала бы скучно: Она сказала…, он сказал….(She/he/they SAID …)
Такие глаголы сообщения помогают точнее выразить, что человек чувствует, его точку зрения или что думает говорящий o чьих-то словах.
Чтобы правильно передать чью-то речь, необходимо использовать глагол сообщения с правильным значением.
Самые распространенные глаголы сообщения – say и tell. У них одинаковое значение – говорить; однако используем мы их по-разному.

Reporting verbs
Существуют еще глаголы сообщения, которые можно использовать вместо say и tell.
Однако с ними нужно быть внимательными при переходе из прямой речи в косвенную, так как эти глаголы влияют на структуру предложения, которое за ним следует.
Вот несколько структур.

David said, “I will take you to the station.”
David promised that he would take me to the station.
Следующая структура отличается! Здесь вам не понадобится правило согласования времен, так как после глагола сообщения нужно использовать ИНФИНИТИВ.
Примечание
После некоторых глаголов можно использовать разные структуры.
